The word is formed regularly from 'commission' + '-er', but the modern office title is somewhat specialized and not fully predictable from the base alone.
Etymology
Commissioner comes from commission, which is made from com- 'together' and mittere 'to send', the same root as mission. So a commissioner is literally someone 'sent' or given a task, which explains why the word now means an official put in charge of a department or area.
