Morphology
The meaning 'an island located in the Caribbean Sea' is directly and compositionally derived from 'Caribbean' (the region/sea/coasts) modifying 'island' (a piece of land surrounded by water). This modifier+head structure is a straightforward, literal compound found cross-linguistically, so a B1 learner who knows both constituents will readily understand the MWE.
Etymology
Caribbean island gets its name from the people called the 'Carib' who lived there when European sailors arrived, so the sea and the islands were called 'Caribbean'. So Caribbean island simply means an island in the 'Caribbean Sea', often remembered for warm beaches and lively music.
