Unfermented is made of un- ('not') and ferment ('to change by yeast or bacteria'), like when grapes become wine or milk becomes yogurt. So unfermented means 'not changed by fermentation', which is why it describes fresh juice, milk, or tea that has not gone sour or become alcoholic.
