Adding the agentive suffix '-er' to the verb 'sympathize' is a regular, productive process yielding 'a person who sympathizes'.
Etymology
Sympathizer comes from the parts sym- meaning 'together' and -path- meaning 'feeling', the same root as in sympathy and empathy. So a sympathizer is someone who 'feels with' a group or cause and so supports its ideas, even if they are not an active member.
