The compound is directly compositional: 'spinal' means relating to the spine or spinal cord and 'fluid' means a flowing liquid, so a B1 learner who knows both words can infer it is a liquid associated with the spine/spinal cord (indeed the cerebrospinal fluid surrounding brain and spinal cord). Etymology
