The sense 'starting at the very beginning' is partly derivable from the parts: 'from' marks origin, 'ground' suggests a foundation/base, and 'up' conveys upward progression, so the construction evokes building upward from a base. However, the mapping from this concrete image to abstract uses (e.g. building a business) and the emphatic sense of 'completely' require a small degree of pragmatic/metaphorical inference, so it is not fully transparent to an unexposed B1 learner.
Etymology
From the ground up comes from building a house: you begin at the ground and build everything up from there, not just fixing one part. So it now means 'starting from the very beginning' and making something complete.
