Morphology
Both constituents directly contribute to the overall meaning: 'dope' = drugs and 'fiend' = an addicted/enthusiastic person, so a B1 learner who knows both words can readily infer 'a person addicted to drugs.' The term is informal/derogatory in register but compositional and cross-linguistically analogous to 'drug addict' constructions, making it transparent.
Etymology
Dope fiend links dope, meaning drugs, and fiend, meaning an evil spirit or a person with a very strong, uncontrollable desire. People used the image of a 'fiend' attacking because the drug craving feels very strong and harmful, so dope fiend came to mean someone 'addicted' to drugs.
