The meaning 'a bar to provide support' is directly and transparently derived from combining 'grab' (to grasp) with 'bar' (a long rigid piece): a bar intended for grabbing. Etymology
Grab bar is very literal: a strong bar attached to a wall that you can grab when you feel unsteady. Because people hold it to avoid falling, it means 'a bar that gives support'.
