Morphology
The meaning is directly compositional: 'old' (earlier/not new) combined with 'testament' (a division of scripture) yields the notion of the earlier/division part of the Bible. This structure is cross-linguistically common (e.g. Spanish 'Antiguo Testamento'), so a B1 learner who knows both constituents would readily infer the MWE's meaning.
Etymology
Old Testament gets its name from calling these sacred books the old 'testament' or promise made before Jesus, to show they come before the New Testament. That's why Old Testament means the books written before Jesus and important to both Jewish and Christian tradition.
